Medical, Dental, and Vision Benefits

We offer a comprehensive plan through Blue Care Network. We offer coverage for the individual with low premium cost. Coverage starts 90 days after the first date of employment.

Financial Security

We provide full-time staff members with basic life, accidental death and dismemberment, short term disability and long term disability insurance.

Retirement Plans

We offer a generous 401(k) plan for all our staff members. Once an employee is eligible to enroll in our retirement program, the agency contributes to the staff member’s account regardless of the staff member’s active participation.

Paid Time Off

We offer a program for full time staff that combines vacation, sick, and personal time off into one category. Full time employees start accruing time upon hire.

Recognized Holidays

We recognize 12 holidays during the calendar year. They are: New Year’s Day, MLK day, President’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, the day after Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, and one Floating Holiday.

Supports Coordinator/Social Work (Part-Time)

Under the general supervision of the Clinical Supervisor, CMD uses person-centered principles to assess the medical, functional, psychological, financial, and environmental needs of MI Choice Medicaid Waiver participants. Works with the participant, family members and caregivers to develop a person-centered plan, to assist and support the participant to manage their care needs and to provide ongoing monitoring and reassessment of participants.

Outreach Specialist

Increase enrollment for the MI Choice Medicaid Waiver Program by conducting targeted outreach. Educate long term care residents, long term care staff and providers, hospital and medical staff, community groups, organizations that serve older adults and individuals with disabilities, and the general public within the service area. Develop working relationships with key referral sources, organizations, groups and individuals that can increase appropriate referrals for the MI Choice Medicaid Waiver Program. Track and report outreach activities and outcomes. Collaborate internally with Community Resources, Eligibility and Care Management staff to solicit ideas for outreach, develop strategies to meet enrollment goals, gauge effectiveness of outreach and report outcomes. Provide input and reports to management as requested. This is a hybrid model (remote, in the community and in the office) after all training has been completed and performance criteria are met.

Information and Referral Specialist

Effectively connect individuals to appropriate resources to address their needs in person, by phone or electronically. Provide education and assist individuals with benefit screening, enrollment and post-enrollment services. Speak at community information sessions, meetings and participate in outreach events to promote agency services and initiatives. Complete applicable documentation, data tracking and reporting. The position is in office. Hourly pay is $16.00 with generous CTO and competitive benefits.
