Increase enrollment for the MI Choice Medicaid Waiver Program by conducting targeted outreach. Educate long term care residents, long term care staff and providers, hospital and medical staff, community groups, organizations that serve older adults and individuals with disabilities, and the general public within the service area. Develop working relationships with key referral sources, organizations, groups and individuals that can increase appropriate referrals for the MI Choice Medicaid Waiver Program. Track and report outreach activities and outcomes. Collaborate internally with Community Resources, Eligibility and Care Management staff to solicit ideas for outreach, develop strategies to meet enrollment goals, gauge effectiveness of outreach and report outcomes. Provide input and reports to management as requested. This is a hybrid model (remote, in the community and in the office) after all training has been completed and performance criteria are met.

Effectively connect individuals to appropriate resources to address their needs in person, by phone or electronically. Provide education and assist individuals with benefit screening, enrollment and post-enrollment services. Speak at community information sessions, meetings and participate in outreach events to promote agency services and initiatives. Complete applicable documentation, data tracking and reporting. The position is in office. Hourly pay is $16.00 with generous CTO and competitive benefits.

I’ve been on staff at The Information Center for thirteen years and I can’t imagine working anywhere else. When I was younger, my parents, Kurt and Ruth Sebaly were always involved in the community and then instilled that desire to serve in the community in me. It is an honor to continue the tradition.
