- Participants will describe MI Choice Medicaid Waiver Program services
- Participants will describe general MI Choice Medicaid Waiver eligibility requirements
- Participants will describe Community Transitions Program services
- Participants will describe general Community Transitions eligibility requirements
- Participants will describe process to refer clients who may be eligible for The Information Center programs.
