Job Description

Increase enrollment for the MI Choice Medicaid Waiver Program by conducting targeted outreach. Educate long term care residents, long term care staff and providers, hospital and medical staff, community groups, organizations that serve older adults and individuals with disabilities, and the general public within the service area. Develop working relationships with key referral sources, organizations, groups and individuals that can increase appropriate referrals for the MI Choice Medicaid Waiver Program. Track and report outreach activities and outcomes. Collaborate internally with Community Resources, Eligibility and Care Management staff to solicit ideas for outreach, develop strategies to meet enrollment goals, gauge effectiveness of outreach and report outcomes. Provide input and reports to management as requested. This is a hybrid model (remote, in the community and in the office) after all training has been completed and performance criteria are met.

Job Responsibilities

  • Establish and maintain working relationships with organizations that can be valuable referral sources for MI Choice Medicaid Waiver including, but not limited to long term care facilities, senior housing, medical providers, dialysis centers, home care agencies, and organizations that serve older adults and persons with disabilities.
  • Develop and implement strategies to meet enrollment goals for MI Choice Medicaid Waiver and promote TIC MI Choice Medicaid Waiver program.
  • Give both formal and informal presentations and be able to answer questions on MI Choice Medicaid Waiver.
  • Perform outreach directly to long term care and housing facility residents and/or their family members.
  • Work with Care Management Department to facilitate positive working relationships and interactions with long term care staff.
  • Participate in community events and fairs as appropriate to educate seniors, persons with disabilities and members of the general public.
  • Participate in community groups to facilitate productive working relationships, general knowledge of community, and ultimately, appropriate referrals to the TIC MI Choice Medicaid Waiver Program.
  • Track and report outreach and enrollment outcomes, analyze outcomes to determine most productive practices and adapt targeted outreach strategies.
  • Present outreach activities, outcomes and strategies regularly to TIC leadership and staff.
  • Maintain strict confidentiality regarding participant and agency information in accordance with HIPAA and other applicable laws and regulations.
  • Other duties may be assigned.

Required Knowledge & Skills

  • Ability to interact successfully with long term care residents, family members, the medical profession, staff of long term care facilities and hospitals, other TIC Home Health Care employees, support coordinators and other members of the community while fostering and maintaining a reputation of excellence and integrity.
  • Ability to communicate both verbally and in writing with supervisors, field staff, office personnel, and participants/families.
  • Ability to do public speaking events to promote MI Choice Medicaid Waiver.
  • Possess excellent customer relation and interpersonal skills.
  • Ability to work with individuals from various backgrounds and establish rapport with a wide variety of professionals and members of the general public.
  • Outgoing and motivated.
  • Ability to multi-task, meet deadlines, and execute work plan independently.
  • Effective time management skills and ability to work independently as well as part of a team.
  • Ability to report for work on a regularly scheduled basis.
  • Ability to successfully provide effective team support for both office and field staff.
  • Ability to lift up to 40 pounds.
  • Possess good working knowledge of MI Choice Medicaid Waiver Program, long term care, and a wide variety of other pertinent community resources.
  • Must have a reliable vehicle.
  • Must possess and maintain valid Michigan Driver’s License and vehicle insurance.

Education & Experience

  • Associates Degree or Bachelor’s Degree in human services or related field preferred.
  • Exceptional communication, public speaking and “people” skills.
  • Knowledge of and contacts within the long term care spectrum within the service area of Out-Wayne County.
  • Demonstrated evidence of applying critical thinking, sensible judgement and critical thinking skills.
  • Demonstrated ability to work closely with others and foster a workable, effective and productive relationship with other staff, administration, volunteers, interns, agencies and consumers.
