MI Choice Medicaid Waiver Vendor Information

Do you represent an established company that serves Wayne or Monroe counties? Are you interested in working with The Information Center to make our community a better place for all of its residents? If so, we would like to hear from you. The Information Center encourages all minority, female, or disability owned businesses to apply.

If you are interested in working with our MI Choice Medicaid Waiver team to provide in home supports and/or other services, please download our Purchase of Service (POS) Package document for information on the application process. 

All MI Choice providers are required to download and review the latest MDHHS Minimum Operating Standards. Changes to the MI Choice program and available service categories will take affect October 1st, 2019. All providers without current copies of the updated MDHHS Operating Standards will be out of compliance. The latest edition can be found here FY2019 MDHHS Operating Standards
