Under the general supervision of the Clinical Manager, CMD uses person-centered principles to assess the medical, functional, psychological, financial, and environmental needs of MI Choice Medicaid Waiver participants in Wayne County excluding Detroit and the surrounding communities. JOB DESCRIPTION: Supports Coordinator/Registered Nurse DEPARTMENT: Care Management FLSA STATUS: Non-Exempt REPORTS TO: Care Management Department Manager
The Director of Care Management provides strategic leadership and administrative oversight for the Care Management Department (CMD) including the MI Choice Medicaid Waiver Program, Community Transitions Program with potential for additional programs for older adults and persons with disabilities. This role ensures all program operations achieve contractual, accreditation, and agency goals while remaining in strict compliance with state and federal standards, including those set by the Michigan Department of Health and Human Services (MDHHS).
The Community Health Worker (CHW) provides hands-on support to MI Choice participants with care transitions, community resource coordination, health education, and follow-up after hospital or nursing facility discharge. The CHW helps participants access services, understand care instructions, manage appointments and medication-related needs, and remain safe and independent in the community. This role works closely with the Supports Coordinator and maintains timely, accurate documentation of all participant contacts and interventions.
Using person-centered practices, assess needs and provide appropriate referrals for clients of multiple programs in a call center environment and community-based settings. Programs include but are not limited to Community Navigation, Michigan Works! One Call, Wayne County Community College Student Assistance Program, and Healthcare Navigation. Complete intake screening and referrals for long-term services and supports programs for older adults and individuals with disabilities. Assist clients with navigating application and enrollment for community and government programs, services and benefits. Provide follow-up, crisis intervention, advocacy and case management as appropriate. This position is in the office and community-based settings with potential to be a hybrid model after all training has been completed and performance criteria are met.

Our office is at 20400 Superior Rd, Taylor, MI 48180. We serve Wayne County and Downriver communities. Hours are Monday through Friday, 9 AM to 5 PM.
