After the transition, many people continue receiving ongoing in-home support through the MI Choice Medicaid Waiver Program, PACE, Home Help, Home Health Care, Habilitation Supports Waiver and Dual Insurance Plans
Help locating and securing a home, apartment, or community-based living setting
Replacing IDs, birth certificates, Social Security cards, and other documents lost or missing after a facility stay
Basic furniture, household items, linens, kitchen supplies, the essentials to make a house a home
Stocked pantry so you have meals ready when you arrive
Rides to your new home and to appointments as you settle in
Security deposits, first month’s rent, and other transition expenses where applicable
Home care, medical services, and community resources you’ll need after the move
A transition navigator stays with you through the whole process
Eligibility
Adults currently living in a nursing facility who want to return to independent or community-based living (home, apartment, assisted living, adult foster care).
Able to live safely in the community with appropriate support.
Located in, or want to move into Wayne, Monroe, Hillsdale, Jackson, Lenawee, Livingston, Oakland, Washtenaw. Counties.
Have Medicaid, are eligible for Medicaid, or have a pending Medicaid application.

A discharge planner, social worker, resident, or family member contacts TIC to start the process.
A TIC transition navigator meets with the resident to understand their needs, preferences, and goals.
We help identify and secure a safe, appropriate place to live based on the resident’s preferences and situation.
We help replace missing documents, gather household items, stock the pantry, and arrange everything needed for day one at home.
We coordinate transportation and support on moving day.
We connect the resident to home-based services and community resources they’ll need after the move.
We stay in touch to make sure the transition is going smoothly and adjust as needed. A transition navigator stays with the resident from the first conversation through settling in at home. You’re not handed off or forgotten.
