MI Choice Medicaid Waiver Program
In-home care services for adults who qualify, so people can stay home instead of moving to a nursing facility. The Information Center coordinates everything: nursing visits, personal care, meals, transportation, home updates, and more.

Free, unbiased counseling on long-term care choices. No sales pitch. No box to fit into. Just a real conversation with a trained counselor about what's out there and what fits your situation. Who it's for: Older adults, adults with disabilities, caregivers, family members, and the professionals who refer them. Cost: Free. No income or asset limits. Service area: All of Michigan's Lower Peninsula.
MI Options - Person-Centered Options CounselingCommunity Transition Services (CTS)
Help for adults moving out of a nursing facility and back to home or community living. We handle the housing search, the setup, the support coordination, and the connections to in-home services — so the move actually works. Who it's for: Adults currently living in a nursing facility who want to transition back to the community.
Community Transition Services (CTS)Community Navigation
A trained Community Navigator helps you find what you need, rent help, utility assistance, food, transportation, mental health resources, legal aid, and more. One call, one person, real answers. Who it's for: Anyone in a moment of crisis or just trying to find the right local resource. Service area: Primarily Wayne County (including Detroit) and Monroe County. We help callers from any area; if you're outside our resource map, we connect you with a Community Navigation agency in your region.

Free job search, training, and workforce help through Michigan Works! One Call. Whether you lost your job, your hours got cut, or you're an employer trying to hire skilled workers, we connect you to the right next step. Help for Wayne County Community College District students with food, housing, utilities, mental health, transportation, and other support, so school doesn't get derailed by life. Common Questions About The Information Center
Anyone who fits the eligibility for one of our services. Most services are free. Some have specific income or geographic boundaries. Call 734-282-7171 and we will help you figure out which program fits.
Most are. MI Options, Community Navigation, Michigan WORKS!, and the WCCCD Student Assistance Program are free. MI Choice and Community Transition Services are funded through Medicaid for people who qualify, so there is no out-of-pocket cost to the client.
Not for most programs. MI Options has no income or asset limits. Community Navigation and Michigan WORKS! are free regardless of Medicaid status. MI Choice and CTS may require Medicaid eligibility, but you do not need active Medicaid to apply.
Many people use more than one. A senior might use MI Options and MI Choice. A job seeker might use Michigan WORKS! and Community Navigation. When you call, we work out which combination fits.
MI Options serves the entire Lower Peninsula of Michigan. The Student Assistance Program serves all WCCCD students. For the other programs, call us anyway. We will help you find the right agency for your region.
Yes. Our programs serve adults 18+, including older adults, adults with disabilities, caregivers, families, and WCCCD students.
