What Is the MI Choice Medicaid Waiver?
MI Choice is a Medicaid program that helps older adults and people with disabilities live safely at home, instead of moving into a nursing home.
It's care coordination, not medical care.
That means The Information Center doesn't replace your doctor or your hospital. What we do is build a plan around YOU and bring in the right people to help.
You don't have to figure any of this out alone. That's what we're here for.
Everyday Help at Home
The day-to-day services that make staying home possible, the kind of help that keeps life moving.
Community Living Supports
Help with everyday tasks like laundry, meals, light housekeeping, shopping, and personal care.
Home Delivered Meals
Nutritious meals brought to your door when cooking is no longer safe or easy.
Adult Day Health (Adult Day Care)
A safe, supervised place during the day with activities, meals, and time with others. A big help for caregivers who work during the day.
Community Transportation
Rides to doctor's appointments, errands, and community events, medical and non-medical.
Chore Services
Help with heavier work around the house, like deep cleaning, yard work, or seasonal chores that have become too much.
Personal Emergency Response System
A wearable button that connects you to help right away if you fall or have an emergency.
Goods and Services
Everyday items that support your care plan and aren't covered through other programs.
Residential Services
Support if you live in a licensed setting, like an adult foster care home or a home for the aged, so you still get the help you need.
Your Care Team and Health Support
The nurses, social workers, and care professionals who get to know you — and stay close as your needs change.
Supports Coordination: Your Supports Coordinator team (a nurse and a social worker) manages your full care plan, checks in often, and adjusts things when life changes.
Nursing Services: Preventive nursing care in your home to help manage health conditions and avoid trips to the hospital.
Private Duty Nursing / Respiratory Care: Specialized nursing for people with complex health needs who require ongoing skilled support.
Community Health Worker: A trained helper from your community who can guide you through health questions, appointment follow-up, and local resources.
Counseling: Someone to talk to. Needing help at home can be a hard adjustment, and you don't have to work through it alone.
Respite: A break for your family caregiver so they can rest and recharge.
Training: Education for you or your family on managing care at home, using equipment, or handling specific health needs.
Tools, Home Updates, and Care Your Way
The equipment, home changes, and self-directed options that let you live the way you want.
Environmental Accessibility Adaptations: Changes to your home like ramps, grab bars, and wider doorways so it's safer to live there.
Assistive Technology: Tools and devices that make daily tasks easier on your own, like special grips, reaching aids, or communication devices.
Specialized Medical Equipment and Supplies: Items like electronic pill dispensers and liquid nutrition that help you stay safe and independent.
Vehicle Modifications: Changes to a vehicle so you can get in and out safely, like wheelchair lifts, hand controls, or modified seating.
Structured Family Caregiving: A program that trains and pays a family member or trusted person to be your caregiver. You get care from someone you already know.
Supports Brokerage: Helps you find, organize, and manage your own care providers if you want more control over your services.
Fiscal Intermediary: If you choose to hire and manage your own caregivers, this service handles the payroll and tax paperwork for you.
Services are based on your eligibility and assessment. Your Supports Coordinator will work with you to decide which services fit your needs.

Do You Qualify?
This is the part where a lot of families stop. They think, "We make too much," or "We don't have Medicaid," and they walk away. Please don't. The rules are more flexible than most people think.
You may qualify if you:
Are age 18 or older with a qualifying disability, OR age 65 or older
Live in Wayne County (this does not include Detroit, Hamtramck, Highland Park, Harper Woods, or the five Grosse Pointes)
Need the same level of care a nursing home provides,but want to stay home or in a community setting
Want to live at home, in an apartment, in assisted living, or in adult foster care
Think you make too much to qualify?
The income and asset limits are more flexible for the Medicaid Waiver Program than most people think.
Having Medicaid, being eligible for Medicaid, or having a pending Medicaid application is helpful, but not necessary to apply for the Waiver Program.
