Ready to Leave the Nursing Facility and Go Home?
You don’t have to stay in a nursing facility if you want to go home.
The Information Center helps adults move from a nursing facility back to a place of their own. We help with finding a home, getting your papers, picking up furniture, stocking your kitchen with food, and giving you a ride.
One of our staff stays with you through the whole move.
You Don't Have to Stay There
Many people in nursing facilities want to go home. A lot of them can.
Some people stay because they don’t know there is help. We are the help.
You may not have a place to live yet. That is OK. We help with that.
You may not have your ID or papers. That is OK. We help with that too.
You may not have furniture or food. That is OK. We bring those things.
You don’t have to figure this out alone.
Getting You Home
The move itself is a lot. We handle the parts that usually get in the way.
A Place to Live. We help you find a home, apartment, assisted living spot, or other safe place that fits you.
Your Papers. We help you replace your ID, birth certificate, Social Security card, and any other papers you don't have.
Move-In Costs. We can help cover your security deposit, first month's rent, and other costs of moving in.
Rides. A ride from the facility to your new home, and rides to important appointments while you settle in.
Making It Yours
Once you are in, we help you make it feel like home, and we stay with you while you settle in.
Furniture. A bed, dresser, table, chairs, lamps, and the basics to make a house feel like home.
Kitchen Items. Pots, pans, plates, forks, towels, and the small things you need every day.
Food. A full pantry so you have meals waiting for you when you walk in.
Care After You Move. We help connect you to home care, doctor visits, and other community help so you keep getting support.
Someone Who Stays With You. One staff person walks with you through the whole move. You will not be passed around.

How Going Home Works
You, a family member, or a staff person at the facility calls TIC. We start the conversation.
A Transition Navigator from Transition Services calls you and determine when to meet in person.
We help you pick a place to live. We make sure it is safe and works for you.
We help replace any ID or paperwork you are missing.
We bring furniture and food. We get the home ready before you move in.
We give you a ride from the facility to your new place. We help you get settled.
After you move, we check in. We help you keep getting support so the move sticks.

It could be an apartment, a small house, an assisted living spot, or another safe home.

No. Moving home does not stop your Medicaid. We help make sure your Medicaid stays active through the move.

Many people we help do not have family close by. One TIC staff person stays with you the whole way.
Yes. Family members often start the call for someone in a facility. We are happy to talk with them.
No. You can live in a facility outside of Wayne County and still move into Wayne County through this program.
