This page is here to help you get the most out of your program. Whether you need to reach your Supports Coordinator, update your care plan, or just understand what’s available to you, we’ve got you covered.

Meals brought to your door
Rides to the doctor, church, or the store
Help with housework, laundry, and chores
A nurse who checks on you at home
Help getting dressed, bathing, and moving around safely
Changes to your home to make it safer (grab bars, ramps)
An emergency button you wear that calls for help
Someone to talk to if you’re feeling down
A day program where you can socialize and get care
Medical supplies you need at home
Respite help (so your caregiver can take a break)
Not sure if a service is part of your plan? Ask your Supports Coordinator. If your needs have changed, we can update your care plan.

Life changes. Your care plan can change with it.
Had a fall? New health issue? Need more help at home? Just call your Supports Coordinator.
We’ll visit you, reassess your needs, and adjust your plan.
You don’t have to reapply. You don’t have to start over. We just update what you already have.
Situations to call about:
Your health has changed
You’ve had a hospital stay or ER visit
You want to move or change your living situation
A service isn’t working well for you
Your caregiver’s situation has changed
You feel like you need more (or less) help
Keep your Supports Coordinator’s number where you can find it easily
If something isn’t working, speak up. We want to fix it.
Let us know about any changes in your health, even small ones
If you go to the hospital, have someone call us so we can help with the transition home
Ask about services you might not know about - like transportation, home modifications, or adult day programs
If your family wants to be more involved, let your coordinator know, we welcome it
If you have Medicaid, keep your Medicaid active. If you have questions about your Medicaid status, call us and we can help

Call 734-282-7171 during business hours (Monday–Friday, 9 AM–5 PM) and ask for your coordinator by name. If they’re unavailable, leave a message and they’ll call you back.
Call your Supports Coordinator and let them know. They can reassess your needs and update your care plan. You don’t have to reapply.
Have someone call The Information Center to let us know. We can help coordinate your care when you come home and make sure your services are ready.
Contact us right away. We can help you understand your options and, in some cases, help you maintain or reapply for coverage.
Yes, if you give permission. Many clients have a family member involved in their care. Let your coordinator know who can speak on your behalf.
