Quick Eligibility Check
Age: 65+ OR 18+ with qualifying disability
Location: Wayne County and Downriver Communities (excluding Detroit, Hamtramck, Highland Park, Harper Woods, Grosse Pointes)
Medicaid: Having Medicaid, being eligible for Medicaid, or having a pending Medicaid application is helpful, but not necessary to apply for the Waiver Program.
Care level: Nursing-home level of care needed
Preference: Wants to remain in home or community setting (home, apartment, assisted living, adult foster care)

Where MI Choice Fits in Your Discharge Planning
MI Choice vs. Nursing Home Placement — MI Choice keeps patients in the community with coordinated home-based care. Appropriate for patients who meet nursing-home level of care but prefer (or can safely live in) a home or community setting.
MI Choice vs. Home Health — Home health is typically short-term, post-acute. MI Choice is long-term, ongoing care coordination. Patients can receive both if eligible.
MI Choice vs. PACE — PACE is an all-inclusive model with day centers. MI Choice is home-based coordination. TIC will refer to PACE when it’s a better fit.
MI Choice + CTS — For patients in a nursing facility who want to transition home, Community Transition Services helps with the move. 50% of CTS clients then enroll in MI Choice for ongoing support.

TIC contacts the patient, verifies eligibility, and begins the assessment process.
A Supports Coordinator (nurse + social worker team) visits the patient, assesses needs, and develops a personalized care plan.
TIC coordinates vendors, arranges services, and begins in-home care. We manage the plan ongoing.

Yes. If the patient wants to transition back to the community, start with a Community Transition Services (CTS) referral. A portion of CTS clients move into MI Choice for ongoing in-home care.
MI Choice is ongoing in-home care coordination for people living in the community.
CTS is a transition program that helps people LEAVE a nursing facility and return to community living.
They often work together, CTS handles the move, MI Choice handles the ongoing care.
